Insurance Information
Here at the Center for Cognitive Behavior Therapy, we are considered "out-of-network" with all insurance companies. Instead of paying a co-pay, clients pay the hourly rate at the time of the visit. We will provide the necessary documentation so that you are able to file with your insurance company for direct reimbursement.
Filing with your insurance on your own is very easy and clients are usually compensated up to 50 to 80 percent after a deductible is met. Of course this depends on your insurance policy. The member claim form for filling a claim on your insurance can be found on your insurance company’s website (typically only a page or two). You may also have the option of filing online. The form requires your general information, and you will find a section called 'session information'. It is in this section you will write 'see attached' and send in the information provided to you by our office. The documentation we provide will have all the necessary information for your insurance company to directly reimburse you.
We do not accept Medicare or Medicaid.
We are committed to providing quality care to individuals in need. If you are unable to see us and are in need of an in-network provider, we will be happy to assist in finding the right provider for you.
